Insurance-Pricing
EZ CARE CLINIC LLC accepts Medicare/Medicaid, most insurance plans, and multiple forms of payment. We highly recommend contacting your insurance company to ensure we’re in their network and confirm if the specific service you seek is covered. For Patients With Insurance
For patients with insurance, your insurance will be billed for the services provided. You will need to pay the copay at the completion of the service. Additional fees may apply for in-office tests. For patients without insurance, all fees will be out of pocket.
General physical exam, preemployment physical will cost $80 out of pocket. We do not do sports physical
Medication refill- $75

Pregnancy test- $30
For patients that have labs ordered, the lab will charge you or your insurance for their services.
Strep test- $30 - For Patients without insurance
For Patients without insurance, we will charge you out of pocket. You will be charged $75 for office visit and for your prescription to be sent to the pharmacy. If you need additional in office test, there will be an extra charge. Virtual visit will be $60. We accept cash payment, debit, credit card, HSA Cards.Rapid Covid- 19 test- $80 (out of pocket)
Rapid vaginitis test- $50
Urine dipstick- $40
Rapid flu A & B test- $90
Blood sugar test- $30
Patient injection admin- $30
Rapid PCR test for travel- $150 Results provided in 1 hour.
Rapid Antigen for travel – $80 Results given within 15 minutes.
